When to Use
When an employee needs to formally report workplace harassment (sexual, physical, or psychological) to their employer's HR department or to the Department of Labour Protection and Welfare.
Key Clauses
Complainant Identification
Full name, employee ID, position, department, and employment start date of the person filing the complaint.
Respondent Identification
Full name, position, and department of the alleged harasser. Relationship to complainant (supervisor, colleague, etc.).
Description of Incidents
Detailed chronological account of each incident: date, time, location, what was said/done, and any witnesses present.
Supporting Evidence
List of attached evidence: screenshots, emails, messages, CCTV references, medical reports, witness statements.
Previous Reports
Record of any prior verbal or written complaints to supervisors or HR, and the response (or lack thereof).
Impact Statement
How the harassment has affected work performance, mental health, and professional relationships.
Requested Action
Specific remedies requested: investigation, separation from harasser, disciplinary action, compensation, policy changes.
Confidentiality Request
Request that the complaint be handled confidentially and protection against retaliation.
Warnings
Keep copies of all complaints and correspondence — do not rely solely on the employer's records.
