Income & Expense Statement
Detailed monthly income from all sources and necessary living expenses to determine repayment capacity.
Proposed Repayment Plan
Percentage of debt to be repaid, monthly payment amount, duration of repayment period, and proposed interest rate reduction.
Creditor Priority Schedule
Order of repayment priority: secured creditors first, then preferred creditors, then unsecured creditors.
Hardship Explanation
